GUEST DOSING INFORMATION




Guest Dosing requires a 1-time setup fee of $20.
This is to get your basic information in our system.

It cost $20 per dose for liquid dosing.
(Tablets or wafers are not available for guest dosing.)

We require a photo ID and confirmation of your Social Security Number.
The confirmation of this number may be given by the "Home" clinic.

The approved list of clinics for guest dosing may be found at: SAMHSA.GOV
The "Home" will also be required to fax guest dosing orders.
Our nursing staff will call to verbally confirm this information.

Due to Arkansas State law requiring a pharmacist:
If take homes are required we may need 7 days advance notice.
Advance notice is required even for Sundays.

If you will be visiting the area for an extended stay your home clinic may require that you provide a random UDS screen.
We CAN provide this service for an additional $20, and will fax the results to your home clinic.
